Are you ready to take the first step to set up an appointment with one of the therapists here at Ann Arbor Counseling Associates? Our goal is to make you feel confortable with this process and take care as much as we can up front so there are no surprises!
What we will need from you...
First we will need to get your basic contact information, payment information, availability, and know a little bit about the goals you have for therapy. You can either call our office at (734) 956-0051 or fill out the form below. If you choose to call the office and plan on using your insurance, please have your insurance card available during the phone call. This information will be required to make an appointment.
Once set up you will be invited to our client portal to fill out your intake forms. Or you can fill out the new client forms by clicking here and sign them in advance so you can jump right in.
*Our offices are open by appointment only. New client appointments can only be made by phone or the form below. There is also no check in person at the office so please arrive on a few minutes early and your therapist will retrieve you from the waiting room.

Client Portal Will Allow You To

We verify your insurance as a courtesy. The client is ultimately responsible for any balances that insurances does not cover.
Any copays, deductibles, co-insurances, or full fees (if you are a private pay client) are due at the time of service.
